Are you interested in purchasing meals for your child?(Required)
We will be making breakfast and lunches in house this year for a cost of $50 per month per child. A sample menu will be available at the bottom of this survey page. You will pay month to month, so you are able to stop or start as needed.

Sports

What sports would you like to see offered at Headwaters?(Required)
We are looking to get one or two sports started this year. Costs would range from $50 - $120 depending on sports.

Clubs

What clubs would you like to see at Headwaters?(Required)
We would like to get a few after school clubs going this year. There would be a small cost involved per club to help purchase items needed as well as pay leaders.
